By signing this agreement, I acknowledge that I am voluntarily choosing to participate in hypnotherapy sessions facilitated by Helen Sherlock. I understand these sessions are a self-exploratory process intended to support personal insight, healing, and transformation.
1. Nature of Hypnotherapy
I understand that hypnotherapy involves guided relaxation and focused attention, often leading to a heightened state of awareness that may support emotional, mental, or spiritual growth. I acknowledge that this is a complementary therapy and not a replacement for medical or psychological care.2. Qualifications of Practitioner
I acknowledge that Helen Sherlock is a qualified practitioner in counselling, community services, and mental health, and is trained in hypnotherapy and spiritual modalities. She is not a registered psychologist, psychiatrist, or medical doctor. I accept that her role is to facilitate the process and that I am responsible for my own wellbeing and decisions throughout.3. Voluntary Participation
I understand that participation is entirely voluntary and that I may decline or withdraw from the process at any time without penalty. While many people benefit from hypnotherapy, individual results may vary, and no guarantees are made regarding outcomes.4. Understanding of Hypnosis
I understand that the effectiveness of hypnosis depends on my openness, willingness, and level of engagement. I acknowledge that resistance may occur and that this is a natural part of the process. I am committed to participating as long as I feel comfortable continuing.5. Spiritual Framework
I understand that Helen Sherlock’s approach is grounded in a spiritual philosophy. Guidance or insights may emerge during sessions that are interpreted as coming from my "Higher Self," "Soul," or related spiritual constructs. I accept that these experiences are subjective and not diagnostic in nature.6. Health, Safety, and Responsibility
If any guidance is received during a session relating to lifestyle, health, or personal choices, I understand that it is my responsibility to evaluate and act upon such information. I agree to consult with a qualified medical or mental health professional regarding any significant health-related decisions. I understand that this service operates in accordance with relevant Australian and Western Australian legislation regarding client safety, privacy, and confidentiality.7. Confidentiality
I acknowledge that all information shared during sessions will be treated with confidentiality, in line with ethical guidelines and privacy legislation, except where disclosure is required by law (e.g. risk of harm to self or others, mandatory reporting obligations).8. By signing below, I confirm that I have read and understood this document, and that I have had the opportunity to ask questions. I consent to participate in hypnotherapy sessions with Helen Sherlock willingly and without pressure or coercion. -
Session Attendance Policy
This policy ensures that sessions are conducted in a safe, confidential, and focused environment by clarifying who is permitted to attend.Sessions are designed to support the individual who has booked, and maintaining these boundaries protects the therapeutic space and upholds professional standards.
Attendance Restrictions
Booking is Non-Transferable:
Sessions are intended solely for the individual who has made the booking.
Bookings cannot be transferred to another person. If the original client is unable to attend, the session must be rescheduled in accordance with the cancellation policy.
No Additional Attendees:
Only the individual who booked the session may be present.
Friends, family members, or support persons may not attend unless prior written agreement has been made for a specific therapeutic purpose (e.g., in family-based or support-involved sessions—these are not part of regular practice and are considered on a case-by-case basis).
This ensures confidentiality, emotional safety, and a focused session environment.
Exceptions
Any exceptions to this policy must be pre-approved by Helen Sherlock in writing before the session takes place.
Requests for joint sessions or support-person attendance will be carefully considered with the client’s goals, therapeutic context, and professional boundaries in mind.
Breach of Policy
If an unapproved attendee is present at the time of the session, the session will be paused or cancelled.
The session fee will be forfeited, and rescheduling will only be possible at the discretion of the practitioner.
Repeated breaches of this policy may result in a suspension or termination of services.
Practitioner Right to Decline Service:
As a holistic practitioner, my priority is to maintain a safe, respectful, and energetically aligned environment for all clients.
In alignment with this, I reserve the right to decline or discontinue services at my discretion and without the obligation to provide an explanation.
This decision may be made in the interest of preserving client wellbeing, practitioner boundaries, or maintaining the integrity of the therapeutic and energetic space.
Clients are encouraged to seek support from another qualified professional should this occur.
Acknowledgment
By booking a session, clients acknowledge and agree to the terms outlined in this policy. These boundaries are essential to creating a respectful, confidential, and effective healing environment.

Session Recording Consent
By signing this agreement, I consent to the recording of my hypnotherapy sessions with Helen Sherlock and acknowledge the following terms:
1. Purpose of Recording
I understand that my sessions may be audio recorded for the primary purpose of supporting my personal development, reviewing session content, and improving service quality. I also acknowledge that recordings may support reflective practice by the practitioner and, with separate consent, may be used for educational or community benefit purposes.2. No Guarantee of Recording Quality
I acknowledge that while recordings are made in good faith, technical difficulties or unforeseen issues may result in incomplete, inaudible, or unusable recordings. I accept that Helen Sherlock cannot guarantee the quality or integrity of every session recording.3. No Refund for Recording Issues
I understand that session fees relate to the professional time and therapeutic service provided, not the recording itself. Therefore, no refund or compensation will be issued for lost or compromised recordings.4. Use of Session Material (Optional & Consent-Based)
I understand that excerpts from my session may be considered for use in educational materials, reflective discussions, or content that supports the broader healing community. This will only occur with my explicit, separate written consentfor each intended use.5. Anonymity and Confidentiality
Should I provide separate consent for my session material to be used for educational or community purposes, I understand that any identifying details (including names, voices, and personal circumstances) will be removed or modified to protect my identity. Helen Sherlock is committed to upholding strict confidentiality in accordance with the Australian Privacy Principles and relevant Western Australian legislation.6. Storage and Security
Recordings will be stored securely using encrypted digital storage systems and will only be accessible to Helen Sherlock or those with express authorised access (e.g., for administrative or IT purposes). Recordings will be retained only for as long as reasonably necessary or as otherwise required by law.7. Acknowledgment

Confidentiality Agreement

By signing this agreement, you acknowledge that you understand your rights to privacy and the limits to confidentiality in accordance with relevant Australian and Western Australian legislation.

I, the client, acknowledge that all personal and sensitive information shared with Helen Sherlock during hypnotherapy sessions will be treated with the utmost confidentiality. This includes, but is not limited to, details about my personal history, emotional experiences, mental health, spiritual insights, and any other information disclosed during the therapeutic process.
2. Limits to Confidentiality
I understand that my personal information will not be shared with any third party without my explicit written consent, except in the following circumstances:

2.3 Risk of Serious Harm
If Helen Sherlock reasonably believes there is an imminent risk of serious harm to the client or another person (including suicide, self-harm, or threats of violence), relevant information may be disclosed to appropriate services (e.g. emergency services, mental health professionals) to protect the safety of all involved.
2.4 Court or Legal Proceedings
If personal records or session content are subpoenaed or otherwise legally compelled by a court, Helen Sherlock is legally obliged to provide the requested information, which may result in a partial or full breach of confidentiality.
3. Use of Anonymous Material for Educational or Public Purposes
Should any material from sessions be used for educational, reflective practice, or public benefit (e.g., case studies, presentations, publications), all identifying details will be removed or altered. Your name, voice, and any personal identifiers will not be used without your specific, separate written consent.
4. Consent to Disclosure for Collaborative Care
If disclosure of session content is requested to support your care through collaboration with another practitioner or service provider (e.g., GP, psychologist, allied health provider), you will be asked to provide written consent before any information is shared.

However, it is important to note that, while I am trained in counselling and hypnotherapy, I am not a psychologist, psychiatrist, or medical doctor. My services do not include the diagnosis or treatment of medical or psychological conditions such as:

Psychiatric disorders (e.g., major depression, anxiety disorders)
Conditions requiring medical intervention
2.2 Client Suitability and Referrals

Substance Use and Session Guidelines
To ensure the safest and most effective therapy, clients must not be under the influence of drugs or alcohol during sessions. This policy is designed to promote clarity, safety, and optimal therapeutic benefit.Substance Use Before and During Sessions
Clients are asked to refrain from the use of drugs or alcohol 48 hours before and after their session to ensure the full effectiveness of the therapy and maintain clarity during the process.Sessions Under the Influence
If a client arrives for a session under the influence of drugs or alcohol, the session will be cancelled without refund. This ensures that both the client and the therapist can engage in the session in a safe and productive manner.Rescheduling Sessions

Refund and Payment Policies

Refund, Payment, and Rescheduling Policy
This policy outlines the terms for payments, refunds, cancellations, and rescheduling of services provided by Helen Sherlock.
1. Payment for Services
Full Upfront Payment: All sessions must be paid for in full at the time of booking via the accepted payment methods provided during the booking process. Your appointment is not confirmed until payment is received.
No Alternative Payment Arrangements: Alternative payment methods or delayed payments are not available.
Multi-Session Packages: For any multi-session packages, payment is required as agreed (e.g., upfront or scheduled installments). If payments are missed or overdue, services may be paused until the balance is cleared.
2. Cancellations or Rescheduling by Helen Sherlock
In the event I am unable to provide a session due to illness, emergency, or unforeseen circumstances:
You will be offered either a full refund, or
The option to reschedule the session at a mutually convenient time.
3. Cancellations or Rescheduling by the Client
Rescheduling: You may reschedule your session with at least 24 hours' notice via the bookings page, subject to availability.
Cancellations with Less than 24 Hours’ Notice: If you cancel within 24 hours of your scheduled session, a 50% cancellation fee will apply. The remaining 50% will be refunded.
No Shows: If you do not attend your session and provide no prior notice, the full session fee will be retained, and no refund will be issued.

All individuals—clients and counsellors alike—are expected to treat each other with respect, dignity, and kindness. Disrespectful language, personal attacks, or inappropriate behaviour will not be tolerated. We ask that all parties engage in a professional manner at all times to foster a conducive environment for growth and healing.Confidentiality

Clients and counsellors are expected to be punctual for scheduled sessions. If a client needs to cancel or reschedule, we kindly ask for at least 24 hours’ notice to avoid any cancellation fees. Repeated lateness or failure to notify of cancellations may lead to rescheduling or other consequences.Non-Discrimination
We are committed to providing a welcoming, inclusive, and non-judgmental environment for all clients. Discrimination on the basis of race, gender, age, sexual orientation, disability, or any other personal characteristic is strictly prohibited. At times, clients may engage in behaviours that are disruptive or inappropriate. The following outlines the process for handling such situations and ensures the maintenance of a productive and respectful environment.
Verbal Warning
If a client exhibits unprofessional behaviour (e.g., disrespectful language, interruptions, inappropriate comments, or physical actions), the counsellor will:Provide a verbal warning to the client.
Explain why the behaviour is inappropriate and how it disrupts the session or affects the therapeutic process.
Clarify the importance of maintaining respectful boundaries to ensure a safe environment for all involved.
Termination of Session
If inappropriate behaviour continues despite a verbal warning, or if the behaviour is severe (e.g., aggression, verbal abuse, or refusal to comply with the professional boundaries set), the counsellor may:Terminate the session immediately, ensuring the client’s safety and well-being.
Attempt to de-escalate the situation if necessary, by offering to reschedule the session and encouraging the client to reflect on their actions.
Discuss the issue with the client privately, with the goal of re-establishing a constructive and respectful environment for future sessions.
Suspension of services may be considered for clients who repeatedly engage in such conduct.
Dismissal from Practice
In cases of persistent unprofessional conduct, or when behaviour continues to disrupt the therapeutic environment despite multiple warnings and session terminations, the counsellor may decide to:Dismiss the client from the practice.
This decision will be made in consultation with the client, with clear reasons provided in writing. The client will be informed of their dismissal and the reasoning behind it, such as repeated violations of the Professional Conduct Policy.
Referral to another professional may be recommended if necessary. The counsellor will help the client identify suitable alternatives for continuing their care.
Handling Specific Behaviours of Concern:
Aggressive or Violent Behaviour
Immediate session termination will occur.
If a client becomes aggressive, physically threatening, or violent, the counsellor will contact authorities if necessary for the safety of all involved.
Substance Abuse or Intoxication
If a client arrives under the influence of drugs or alcohol, the session will be terminated and rescheduled.
A referral to appropriate services for substance abuse treatment may be provided if needed.
Failure to Respect Boundaries
If a client violates professional boundaries (e.g., inappropriate comments, personal requests), they will be reminded of the policy.
Continued disregard of boundaries may lead to the termination of services.
